The Philippines is among the countries most vulnerable to the impacts of climate change, facing increasing risks from extreme heat, typhoons, flooding, sea-level rise, and other climate-related hazards. These hazards have significant implications for public health, including increased injuries and deaths, climate-sensitive diseases, food and water insecurity, and disruptions to essential health services. Strengthening the resilience of communities and health systems is therefore critical to protecting health and reducing climate-related vulnerabilities. Pursuant to Republic Act No. 9729 (Climate Change Act of 2009) and Republic Act No. 10121 (Philippine Disaster Risk Reduction and Management Act of 2010), the Department of Health (DOH) is advancing the development of a Health National Adaptation Plan (HNAP) to integrate climate change adaptation into health sector planning. A comprehensive Climate and Health Vulnerability and Adaptation (V&A) Assessment is needed to identify priority climate-sensitive health risks, vulnerable populations, geographic hotspots, and health system gaps. The assessment will provide the evidence base for targeted adaptation actions, investment planning, and the development of climate-resilient health systems at national and subnational levels. In line with this, WHO is seeking an institutional contractual partner to provide technical assistance on the Conduct of Climate Change and Health Vulnerability and Adaptation (V&A) Assessment in Support of the National Adaptation Plan (HNAP), Philippines.

EUR from public and private investors). The objective is (1) to build a strong pipeline of IsDB projects in the three countries that are adapted and/or with a main objective of addressing adaptation challenges in relevant sectors, building from PACO small grant projects (Output 3), and NAP/NDC investment plans developed or revised under Output 1, and (2) to facilitate capacity building to strengthen the institutional capacity of financial institutions. The contractor will be responsible, in coordination with IsDB and GIZ, for deploying a series of country-level consultation workshops to firm up and engage with relevant public and private actors for the strengthening of IsDB project pipelines in Benin, Ivory Coast and Senegal with high adaptation finance potential. Establishment of four Reverse Linkages and one centre of excellence in collaboration with IsDB This work package focuses on the establishment of four Reverse Linkages in the PACO program with four different host-institutions. Reverse linkages are a mechanism developed by the Islamic Development Bank to establish partnerships between institutions in countries in the global South. In each partnership an institution with proven knowledge, expertise, technology, and resources is the provider of knowledge; the recipient of knowledge can be a public, private, or third-sector organization. The institutions involved must be IsDB members; this limits PACO participation to Benin, Ivory Coast and Senegal, and to the CCRS , since the Republic of Congo is not a member of the IsDB. The contractor will support IsDB to conduct a stakeholder assessment in each of the three member countries to identify interest and needs among institutions in the three target countries in being the recipient organization in a reverse linkage relationship. Where this offers a good opportunity, the contractor will, with help and consent of the IsDB, identify knowledge-providing institutions (the "Resources-Centre") to work with the concerned institutions and will facilitate planning and establishing the partnerships. The resulting partnerships may offer a range of services to recipient countries, including training, coaching, advisory services, technology transfer, demo sites, and others. Once set up, the partnerships usually last for a period of three years, so the process will be carried out during the life of the PACO project. Declining global health budgets and fragmented funding structures place additional pressure on national systems to use available resources more efficiently and to integrate externally financed measures into domestic health and financing frameworks. BACKUP Health supports governments and key actors in addressing these challenges by improving the coordination between global and national processes and by strengthening systemic capacities for planning, governance and implementation. The programme aims to improve the efficiency of globally financed health services and products, particularly for HIV, Tuberculosis and Malaria (HTM), in selected partner countries. BACKUP Health works in close collaboration with ministries of health and finance, national coordination mechanisms, civil society organisations, and regional actors, particularly in countries in Africa and Central Asia. It promotes gender equality, human rights, and the inclusion of vulnerable and marginalised groups as cross-cutting priorities. In the upcoming phase (2026-2028), the programme provides technical and policy support in three core areas: - Health financing and governance: Strengthening institutional capacities for the integration of global, particularly HTM-related, financing into national health systems and governance structures. - Integration of services: Supporting partner countries in developing and implementing approaches for integrated, people-centred and equitable health services, with strong community engagement. - Local production: Enhancing organisational and institutional capacities for the local manufacturing of HTM-relevant health products in Africa to improve sustainability, supply security and regional self-reliance. The project is implemented with a focus on partner countries in Africa and Central Asia. With this tender, BACKUP Health calls for assistance in the swift formulation and/or management of technical assistance (TA) assignments in countries eligible to BACKUP Health support. Technical assistance assignments may cover activities such as - Advisory support on sustainable health financing and governance, including domestic resource mobilisation, public financial management, and alignment of Global Fund and other external resources with national systems, - Technical support for integrating HIV, TB and malaria (HTM) services into broader, people-centred health systems through cross-programmatic collaboration and strengthened service delivery, - Support for building local and regional capacities for health product manufacturing, regulation, and market development. Assignment process Requests for technical assistance can be expected on a rolling basis throughout the project duration (2026-2028). It cannot be specified in advance which partner countries will request support, how frequently such requests will be submitted, or which thematic areas will be prioritised. BACKUP Health will forward TA requests to the contractor, who shall then identify, hire and deploy suitable experts to provide the requested services across the three support areas mentioned above. Deutsche Gesellschaft für Internationale Zusammenarbeit (GIZ) GmbHEschbornNepal has made significant progress in improving the overall health status of the population, with huge improvements in life expectancy and reductions in maternal and child mortality rates. However, access to quality healthcare remains unequal, as many citizens face barriers due to geography, socio-economic status, and economic constraints. Nepal National Health Accounts (NHA) report indicates that out-of-pocket (OOP) healthcare expenses accounted for 54.2% of total health spending in 2019/20, exceeding the South Asian average of 48.2% (WHO 2023). Government contributions to health expenditure are only 30%, which is lower than the regional average of 42% (WHO GHED 2023). A 2018 study found that 10.3% of households faced catastrophic health expenditures monthly (Ghimire et al. 2018). Furthermore, the report found that 10.7% of households spent 10% of their total household expenditure on health and 2.4% even spent 25%, implying that Nepalese households face increased vulnerability to catastrophic health expenditures risking impoverishment (ADB 2024). To achieve Universal Health Coverage (UHC), Nepal seeks to reduce OOP spending through the National Health Insurance (NHI), launched in 2016. This initiative, overseen by the Health Insurance Board (HIB), aims to provide universal access to quality health services, primarily for the informal sector. Currently, the NHI is operational in all 753 local structures across 77 districts. Though the Health Insurance Act 2017 has clearly mentioned the health insurance to be mandatory, in practice this is not yet enforced; highlighted by the fact that out of 8.1 million ever-enrolled, currently only 4.4 are actively insured as of June 2024. The increasing coverage and expansion of the NHI is a priority to the Government of Nepal (GoN) and strengthening the institution has been identified as an important action area within the strategies in both the National Health Financing Strategy (NHFS) 2022-2032 and in the National Health Sector Strategic Plan (NHSSP) 2022-2030. GIZ has been supporting the national health insurance program since 2013, on policy advisory aspects as well as technical aspects. To strengthen HIB, GIZ supported a pool of experts through the Support to Health Sector Strategy (S2HSS) program in 2023. The pool of experts supported in defining roles and responsibilities of the policy, planning and monitoring department (PPMD) and developing job descriptions for those assigned to the departments, as envisioned in the Organisation and Management (O&M) survey. In addition, the support was extended for development of crucial policy documents, strategies, guidelines, and plans. On a technical front, GIZ supported HIB for improving the open-source insurance management information system (openIMIS) to streamline core business processes: from enrollment, member verification, policy activation to claims processing and reimbursement. Meanwhile, in the current context of HIB with constrained institutional capacity despite having developed numerous strategic documents in 2023, prioritizing its execution along with implementation of necessary reforms is now critical. Additionally, increasing population coverage, contextually redefining the benefit package, enhanced claims processing, strategic purchasing, actuarial analysis for decision modelling, using data analytics for enhancing quality of care and coordination with other actors like the Social Security Fund (SSF), Employee"s Provident Fund (EPF) etc. is vital to improve efficiency and avoid redundancy in successful implementation of the health insurance program. In order to address these policy, strategy and operational level challenges, GIZ calls for a team of nine short-term and long-term experts with diverse expertise for building up the health insurance to strengthen the institutional capacities of the HIB and other social protection actors to implement the national health insurance program in Nepal. The team will consist of 2 international experts on organizational management and health insurance, 1 short-term national senior expert on health insurance, 4 national short-term experts (covering the topics insurance law, health economics, quality management and actuary analysis) and 2 national long-term experts (health economist and Monitoring & Evaluation specialist). The contract shall be managed and overseen by an international team leader and supported by a national team leader. The period of assignment of the contract will be 01 April 2025 to 31 March 2026. The tasks to be performed by the contractor have been briefly listed in the following four work packages: Work package 1: Support HIB in setting the normative and strategic structure and functions for refining the health insurance program, especially related to financial sustainability aspects. Work package 2: Harmonization of procedures and service packages of national health insurance program and other programs offering benefits in the event of illness. Work package 3: Organizational and institutional capacity development measures to improve core processes of the health insurance program. Work package 4: Provide advice to HIB on various technical matters, focusing on use of data, data analysis, evidence-based planning, and quality management. By the end of the contractual period, the experts need to develop five strategy papers relating to the financial sustainability of the national health insurance scheme, and adequately capacitate the health insurance board for major policy reforms as well as for efficiently enhanced operational functions of the program. The contractor should conduct workshops, meetings, policy dialogues etc. for the purpose.
