Mit dieser Ausschreibung wird eine Anbieterin gesucht, welche die Interdisziplinäre Fallkoordination und die aufsuchende Beratung von Menschen mit Demenz ab dem 1. Januar 2027 kantonal zentral leitet, diese Bereiche effizient und kostensensibel neu organisiert und die Dienstleister im Gesundheits- und Sozialwesen entlastet. Schwerpunkte des Angebotes sind die Information, Koordination, Triage und Beratung in komplexen Lebenssituationen bzw. Krankheitsverläufen von älteren Menschen sowie von Menschen mit Demenz.
