L’adjudicateur déploie un programme de réadaptation gériatrique à domicile via le CMS RéaDOM, fondé sur une approche interprofessionnelle associant infirmiers, ergothérapeutes et physiothérapeutes.Le programme RéaDOM vise à soutenir le maintien à domicile de personnes âgées présentant une perte d’autonomie fonctionnelle, à travers une prise en charge coordonnée, intensive et limitée dans le temps.Dans ce cadre, l’adjudicateur lance un appel d’offres en vue de sélectionner un prestataire de services en physiothérapie en mesure de répondre au cahier des charges suivants ;· des prestations de physiothérapie à domicile ;· une collaboration étroite et structurée avec les équipes d’infirmières et d’ergothérapie ;· une participation active à la coordination clinique du programme RéaDOM ;· Un interlocuteur unique pour le Responsable du Centre RéaDOM.
